2. You would need a notarized Affidavit on a non-Judicial Stamp Paper of 10rs, in case you take a gap between your 12th examination and MBBS Admission.
3. UP NEET registration form, Admit Card of NEET, and NEET Scorecard.
4. Also, you should have a seat allotment letter that you would have received from the counseling.
5. Domicile Certificate.
6. Proof of address Aadhar Card/ Election Voting Card/ Passport/Driving license, etc.
7. Caste Certificate (if applicable)
8. You should also bring 15 passport-size photos of yours and one passport-size photo of each of your Mother and Father
9. You would also need Migration Certificate/ Transfer Certificate
