UP NHM CHO Online Form 2024 is available on its official web portal.  It is the 6-Month Bridge Program (Certificate) in Community Health under NHM-UP. As per the latest announcement, the UP NHM CHO Application Form process will be commenced from ie 9-March-2024 and will go end on 23-Mar-2024. All candidates are advised to complete the registration process before the last date of the exam. Only online applications will be accepted by the conducting authority.

Steps to fill the UPNHM CHO Online Form 2024:

Candidates should read the guidelines/instructions carefully before filling out the form. Follow the below steps for completing the online form.  

Step 1: Go through the //upnhm.samshrm.com link, and start your application process. As you click on that link you will be reached on a new web page. There will be two types of users.

Step 2: If you did not register yet then click on New Registration and if you are a registered user, then click on Already Registered and enter Login ID & Password to proceed.

Step 3: Before moving to further, candidates should ensure that they are meeting all the eligibility criteria for which they are applying.   

Step 4: When the registration has been successfully completed, candidates will get an email comprising of User Id and Password. These credentials will be required for the aspiring candidate for applying to UPNHM CHO 2024 job.

Step 5: After login into your account on the site, a UPNHM CHO Online Form 2024 will be available. Here, applicants need to enter his/her personal and academic details like Date of Birth (DOB), Address, State, Mobile Numbers, Email, etc. very carefully along with other mandatory details.

Step 6: Afterwards, candidates have to upload the scanned documents in the prescribed format.
